The new treatment criteria and studies discussed in the video mainly concern adults, so a specialist’s guidance takes priority for children and adolescents.

Waiting for the test results

Recently, my daughter had a blood test for hepatitis B.

Worried about what the results might show, I watched Doctor Friends’ video If You Have Hepatitis B, You Must Watch This: A Treatment That Reduces Liver Cancer.

Terms such as ALT, HBV DNA, viral load, liver fibrosis, and functional cure appeared throughout the video.

I knew they were important, but it was not easy for a parent waiting for results to know what to examine first.

I brought home a copy of the report and decided to understand what each real number meant instead of worrying vaguely.

The results arrived

The ultrasound findings were the first thing that caught my eye in the report.

The upper abdominal ultrasound found no focal liver lesion, bile duct dilation, or ascites, and no remarkable finding in the liver, gallbladder, spleen, or visible pancreas.

It also identified no new abnormality compared with the previous ultrasound.

The median liver stiffness was 4.9 kPa, and the median CAP value for fat was 201 dB/m.

The CAP value was below the report’s reference cutoff of 250 dB/m for the first stage of fatty liver.

Albumin 3.9 g/dL, total bilirubin 0.6 mg/dL, gamma-GT 12 IU/L, alkaline phosphatase 56 IU/L, and total protein 7.5 g/dL were also within the report’s reference ranges.

In simple terms: Liver stiffness estimates how firm the liver has become, while CAP estimates how much fat has accumulated.

It was reassuring that the ultrasound showed no structural abnormality and that the report did not suggest clear fatty liver or impaired liver function.

However, a liver stiffness value of 4.9 kPa alone cannot establish a fibrosis stage in a child.

Cutoffs are affected by age, equipment, test method, and liver inflammation, so a specialist must interpret them.

Some numbers still needed careful follow-up.

In simple terms: AST and ALT can show liver cell injury, while HBV DNA shows how actively the virus is multiplying.

In simple terms: HBeAg was detected while its corresponding antibody, anti-HBe, was not, and doctors use this result with HBV DNA and other tests to assess viral replication.

Taken together, these results showed very active viral replication and an elevated ALT signal of liver cell injury.

Even so, one test alone could not establish that treatment was required.

A normal ultrasound did not mean everything was over, and a high viral load did not mean treatment was immediately decided.

We needed to compare previous and future tests to see whether the changes persisted.

What the video helped me understand

Hepatitis B involves both the virus and the immune response

Hepatitis B is an infection of liver cells by the hepatitis B virus.

In Korea, many chronic infections historically occurred around childbirth when the virus passed from mother to baby, which is called perinatal or vertical transmission.

It is not a genetic disease, and it did not happen because a child or parent did something wrong.

Having the virus in the liver does not always cause severe symptoms immediately.

Inflammation and injury can occur when the immune system responds to virus-infected liver cells.

If this continues, scar tissue called liver fibrosis can accumulate and eventually increase the risks of cirrhosis and liver cancer.

One liver number cannot tell the whole story

The biggest change in my understanding was realizing that no single number describes the entire condition.

A normal ALT does not mean the virus is absent or guarantee that no future problem will occur.

Conversely, a high HBV DNA level alone does not prove severe liver damage.

Age, changes over time, HBeAg status, and liver fibrosis must be considered together.

Children should not be judged by adult criteria

The 2026 Korean Association for the Study of the Liver guideline recommends antiviral treatment for adults with moderate viremia, defined as HBV DNA from 2,000 IU/mL through 10⁸ IU/mL, regardless of ALT.

This change gives viral load a more active role in treatment decisions than before.

In simple terms: Adults once often waited for liver enzyme levels to rise, but treatment may now begin when the virus is detected within this range even if those levels are normal.

My daughter’s 8.5×10⁷ IU/mL falls within that numerical range, but adult criteria should not be applied directly to an adolescent.

Treatment for children and adolescents may be considered when the following conditions are assessed together.

For children and adolescents, doctors also consider disease progression, the possibility of long-term treatment, adverse effects and resistance, age, and weight.

ALT can also rise for reasons other than hepatitis B, so it matters how long the ALT 71 has persisted and how it changes at the next test.

Treatment criteria are becoming more proactive

In the past, treatment often did not begin unless ALT rose enough even when HBV DNA was high.

The new guideline moves toward considering viral load and long-term risk more actively instead of waiting only for liver enzymes.

The ATTENTION study introduced in the video was one piece of evidence behind this change.

In an interim analysis of 734 adults aged 40 to 80 with chronic hepatitis B and no cirrhosis, the tenofovir alafenamide (TAF) group had about 79% fewer combined events of liver cancer, liver decompensation, transplantation, and death than the observation group.

In simple terms: Serious liver-related problems occurred less often in the adults who started medication early, but this does not mean a child would have the same result.

The outcome combined more than liver cancer alone, and the total event count was small at two in the treatment group and nine in the observation group.

It was an interim analysis of an ongoing study whose participants were all adults, so it cannot be applied directly to my daughter.

It still matters because it suggested that earlier treatment may benefit some adults with detectable virus even when liver enzymes are not high.

Treatment can protect the liver even now

Oral antiviral medicines such as entecavir and tenofovir strongly suppress viral replication.

When taken appropriately, they can lower HBV DNA to undetectable levels and help reduce the risks of liver injury, cirrhosis, and liver cancer.

They do not completely erase every trace of the virus, but methods to suppress it and keep the liver healthy already exist.

A specialist chooses the medication and treatment timing after considering kidney and bone health, age, pregnancy potential, and treatment history.

Treatments aiming for a functional cure are getting closer

A sustained disappearance of hepatitis B surface antigen from the blood is called a functional cure.

It is uncommon with current standard therapy, but treatments targeting it directly have now reached late-stage clinical trials.

In simple terms: This does not mean every trace of the virus has been removed from the body, but that surface antigen remains absent after treatment ends.

Bepirovirsen is a once-weekly injection that remains under research and regulatory review.

Two phase 3 trials reported in 2026 enrolled adults already receiving nucleos(t)ide analogue therapy who had no cirrhosis and met the studies’ surface antigen criteria.

At week 72 after 24 weeks of treatment, 20% and 19% met the functional cure endpoint in the two trials, compared with 0% in the placebo groups.

These figures are not a cure rate for every patient, and the treatment is not immediately available for adolescents.

Future treatments should never become a reason to delay tests or therapy needed now.

Even so, it was a clear source of hope that hepatitis B treatment is moving beyond suppression toward functional cure.

Why the future felt brighter

I still had worries after receiving the results.

Even so, what I understood from the video made the future feel brighter than before.

I do not want to assume everything is over because of one normal result or imagine the worst because of one high number.

We will keep comparing results and watching which direction the virus and liver condition take.

For now, regular testing is still necessary while specialists decide when treatment should begin.

But risks can be detected earlier, the virus can be suppressed, and treatments aiming for a functional cure are getting closer.

Instead of remaining vaguely afraid, I will help my daughter enjoy her life and wait with her for the day she overcomes hepatitis B.

In one line: Hepatitis B treatment criteria are advancing, and therapies aiming for a functional cure are getting closer.
